EXCEPTION REVIEW FORM

  • Handheld (E-Pod) – You must enter all damage codes along with attaching pictures of damage into your handheld device.
  • Exception form must be filled out completely for all damages regardless of how minor you think it is.
  • Not all damages are the fault of the driver so it is also in your best interest to document all damages thoroughly and promptly.
  • Fill out this form as soon as you can while you still have the information at hand.
  • Failure to complete this form could result in damage automatically being charged to driver.
  • Driver must call in to report all damages to the Manager of Claims, if not available leave a voicemail with details of the damage. You may also email your notification along with pictures to claims@proautotran.com or Direct Line (904) 330-0770.
  • Remember, honesty is always the best policy; follow company and manufacturer policies, complete proper inspections
