Handheld (E-Pod) – You must enter all damage codes along with attaching pictures of damage into your handheld device.
Exception form must be filled out completely for all damages regardless of how minor you think it is.
Not all damages are the fault of the driver so it is also in your best interest to document all damages thoroughly and promptly.
Fill out this form as soon as you can while you still have the information at hand.
Failure to complete this form could result in damage automatically being charged to driver.
Driver must call in to report all damages to the Manager of Claims, if not available leave a voicemail with details of the damage. You may also email your notification along with pictures to claims@proautotran.com or Direct Line (904) 330-0770.
Remember, honesty is always the best policy; follow company and manufacturer policies, complete proper inspections
